Hip Osteoarthritis
Also called arthritis of the hip: exercises and advice to manage the early stages without an operation
Osteoarthritis is a common cause of pain in the hip and groin in later life. In the early stages, there is a lot you can do yourself to ease the pain and keep active, and regular exercise is the most important part. This page explains what osteoarthritis is and what helps, and sets out the exercises step by step. If the arthritis becomes severe, a total hip replacement is a very successful operation for hip osteoarthritis.
Understanding hip osteoarthritis
The hip is a ball-and-socket joint: the ball at the top of the thigh bone fits into a socket in the pelvis. In a healthy hip, both are covered with smooth, slippery cartilage, so that the joint moves easily. In osteoarthritis, this cartilage slowly becomes thinner and rougher, the bone underneath thickens, and small bony bumps can form at the edges of the joint. The hip becomes stiffer and harder to move.
The pain is usually felt in the groin, and sometimes in the buttock, the thigh, or even the knee. It is often worse when walking, on stairs, and getting up after sitting, and the hip feels stiff after resting. Putting on socks and shoes, cutting your toenails, and getting in and out of a car can become difficult, and you may limp. It is more common after the age of 50, in people with a family history of it, after a hip problem in childhood, and after years of heavy work, such as farming or heavy lifting.
Osteoarthritis is not simply wear and tear. The hip is living tissue that responds to how it is used: keeping it moving, and the muscles around it strong, helps it, and exercise does not damage it. The pain does not always match what the X-ray shows, and it often comes and goes, with good and bad spells. In some people the hip stays much the same for years; in others it gets worse more quickly.
This page is about osteoarthritis. Other conditions can also damage the hip, such as a loss of blood supply to the ball of the hip (avascular necrosis) or inflammatory arthritis, and they are treated differently: Dr Malik will tell you which you have.
What helps most
- Exercise. Strengthening the muscles around the hip, keeping it moving, and staying active, is the treatment that every major guideline puts first, and it is safe for the joint. How much it eases the pain varies from person to person: a lot for some, and only a little for others.
- Keeping to a healthy weight. If you are overweight, losing some weight takes load off the hip, can make walking and everyday activities easier, and lowers the risks of any later operation.
- Understanding the condition, and pacing your day, so that you keep doing what matters to you.
Keeping active
- Walking is one of the best things you can do. Build up gradually to about 30 minutes on most days: it can be split into shorter walks.
- If walking hurts, an exercise bike, swimming or exercising in water, and tai chi are gentler on the hip, and are good alternatives. On a bike, set the seat high enough that your hip does not bend too far. If you do yoga, leave out poses that bend or twist the hip a long way.
- Break up long spells of sitting by getting up and moving about regularly.
- Pace yourself: spread heavy or long jobs through the day, and alternate standing tasks with sitting ones. Do not push through severe pain.
Everyday tips
- A walking stick, held in the hand on the opposite side to the painful hip, takes load off the hip and makes walking easier. The handle should be level with your wrist when your arm hangs down.
- On stairs, use the rail. Going up, step up with the less painful leg first; coming down, step down with the painful leg first.
- Sit on a firm chair with arms, high enough that your knees are not above your hips: low, soft seats are hard to get up from. Squatting and sitting cross-legged on the floor bend and twist the hip a long way. If they hurt, use a chair, a raised (Western-style) toilet or a toilet frame, and a stool for prayers or chores.
- Wear comfortable shoes with a cushioned sole. If bending down to your feet is hard, slip-on shoes, a long-handled shoehorn and a sock aid help.
- To get into a car, sit down on the seat first, then swing both legs in together.
- If the hip aches at night, lie on your back, or on your other side with a pillow between your knees.
- Keep your bones healthy: get enough calcium and vitamins, especially vitamin D3, and ask Dr Malik whether you need a bone density check.
Easing the pain
- A hot pack or an ice pack, wrapped in a thin towel, for 15 to 20 minutes, can ease the pain: use whichever helps you more, and check the skin regularly.
- Painkilling tablets, taken as prescribed by Dr Malik, can ease the pain enough to keep you active. Gels rubbed on the skin are less likely to help the hip, because the joint lies deep.
- Supplements such as glucosamine can help some people, but not everyone.
- A steroid injection into the hip can ease the pain for a while in some people. As the hip lies deep, it is given either in the operating theatre, guided by X-ray, or by a radiologist, guided by ultrasound. How much it helps, and for how long, varies from person to person: Dr Malik will talk it through with you, and the choice is yours. He gives no other injections into the hip, such as PRP.
- If a hip replacement is planned, a steroid injection is not given in the 3 months before the operation, because it may raise the risk of infection.
- During a flare-up, cut back on walking and the harder exercises for a few days, keep doing Part 1 gently, and build up again as the pain settles.
If a flare-up does not settle, or you are worried about your hip, contact the clinic on +91 99197 03477.
Contact the clinic if
- The pain is getting steadily worse despite the exercises, or it keeps you awake at night.
- You can walk less and less far, or your limp is getting worse.
- Everyday activities, such as putting on socks and shoes, stairs or getting up from a chair, are getting harder and harder.
Contact the clinic urgently if
- Your hip suddenly becomes much more painful, so that you cannot put weight on the leg, or you feel feverish or unwell.
- After a fall, you cannot stand or put weight on the leg, or the leg looks shorter or turned outwards.
- You have pain or swelling in your calf.
Clinic: +91 99197 03477. If you are short of breath or have chest pain, go to the nearest emergency department straight away.
How to do the programme
- Warm up first with 3 to 5 minutes of gentle walking, and finish the same way.
- Do Part 1 every day, once or twice a day. These gentle exercises keep the hip moving, and are the ones to keep doing in a flare-up.
- Do Part 2 three times a week, with a day’s rest in between: the muscles get stronger on the rest days. When Part 2 feels easy, add Part 3 on the same days.
- Move slowly and with control: lift slowly, hold, and lower slowly. The last few repetitions should feel like hard work. When an exercise feels easy, make it a little harder, as the card suggests.
- If both hips are painful, do the exercises with both legs. If you have had a hip replacement on the other side, ask Dr Malik which exercises suit that leg.
- Give it time: it usually takes a few weeks of regular exercise before you notice a difference, and the benefit lasts only as long as you keep exercising.
Before you begin
- This programme is for the early stages. If the arthritis becomes severe, a total hip replacement is a very successful operation. Trying exercises first does not rule out a replacement later.
- If you have heart or lung problems, or problems with your balance, check with Dr Malik before you start.
- Some aching or discomfort during the exercises is normal, and does not harm the hip. Do a little less next time if the pain is much worse than usual during an exercise, has not settled within about 2 hours, or the hip is more painful the next morning; then build up again gradually.
- When you exercise standing up, keep a kitchen worktop or a sturdy chair within reach for support.
- Follow these exercises as they are written unless Dr Malik tells you otherwise.
- This information does not replace individual advice. If it differs from what you were told in clinic, follow the advice you were given there.
Part 1: Moving the hip
Every day, once or twice a day. Gentle exercises to ease stiffness and keep the hip moving: keep doing these during a flare-up.
Bending the hip and knee, lying
Keeping the hip moving, which eases stiffness.
- Lie on your back on a bed, with your legs straight. A plastic bag under your heel helps it to slide.
- Slide the heel of your painful leg up towards your bottom, bending the hip and knee as far as is comfortable. Hold.
- Then slide the heel back down until the leg is straight again, and relax.
- Hold5 seconds
- Repetitions10
- How oftenOnce or twice a day
Sliding the leg out to the side
Keeping the hip moving outwards, which you need for walking and for getting in and out of a car.
- Lie on your back on a bed, with your legs straight and your toes pointing up.
- Slide your painful leg out to the side as far as is comfortable, keeping the knee straight and the toes pointing up. Hold.
- Slide it back to the middle, and relax.
- Hold5 seconds
- Repetitions10
- How oftenOnce or twice a day
Rolling the knees from side to side
Gently turning the hips in and out, which eases stiffness.
- Lie on your back with your knees bent, and your feet flat on the bed and together.
- Keeping your shoulders flat, let both knees roll slowly to one side, as far as is comfortable.
- Bring them back to the middle, then roll them to the other side.
- Repetitions10 each way
- How oftenOnce or twice a day
Lying on your front
Stretching the front of the hip, which gets tight with sitting. It helps you to stand up straight and walk with a longer step.
- Lie on your front on a firm bed, with your arms by your sides or your head resting on your hands.
- Let the front of your hips relax down onto the bed, and stay there.
- If you cannot get onto your front easily, or it hurts your back, leave this one out.
- TimeA few minutes, building up to 10
- How oftenOnce a day
Part 2: Building strength
Three times a week, with a day’s rest in between. Start with 1 set of each and build up to 3 sets of 10. Move on to Part 3 when these feel easy.
Bridging
Strengthening the buttocks, which steady the hip when you walk and climb stairs.
- Lie on your back with both knees bent and your feet flat on the bed or the floor.
- Squeeze your buttocks and lift your bottom up, until your body is in a straight line from your shoulders to your knees.
- Hold, then lower slowly. When this is easy, do it on your painful leg only, with the other leg held straight.
- Hold5 seconds
- Repetitions3 sets of 10
- How often3 times a week
Opening the knees, lying on your side
Strengthening the muscles at the back and the side of the hip.
- Lie on your side with your painful hip on top, your hips and knees bent, and your feet together.
- Keeping your feet together, lift your top knee as far as is comfortable, without letting your hips roll backwards. Hold.
- Lower it slowly. When this is easy, tie a stretchy exercise band around your thighs, just above the knees.
- Hold5 seconds
- Repetitions3 sets of 10
- How often3 times a week
Lifting the leg out to the side, standing
Strengthening the muscles at the side of the hip, which keep your pelvis level when you walk.
- Stand upright, facing a kitchen worktop or the back of a sturdy chair, and hold on to it.
- Keeping your knee straight and your toes pointing forwards, lift your painful leg out to the side. Do not lean over.
- Hold, then lower it slowly. Then do the same with the other leg.
- Hold5 seconds
- Repetitions3 sets of 10
- How often3 times a week
Taking the leg backwards, standing
Strengthening the buttocks, which help you to walk and to stand up straight.
- Stand upright, facing a kitchen worktop or the back of a sturdy chair, and hold on to it.
- Keeping your knee straight and your body upright, take your painful leg backwards, a short way only. Do not lean forwards or arch your back.
- Hold, then bring it back slowly. Then do the same with the other leg.
- Hold5 seconds
- Repetitions3 sets of 10
- How often3 times a week
Standing up from a chair
Strengthening the hips and thighs for getting up, stairs and walking.
- Sit near the front of a firm chair, with your feet flat on the floor, slightly apart.
- Lean forwards over your feet and stand up, using your hands on the chair only if you need to.
- Sit back down slowly, with control. When this is easy, do it without your hands, and then more slowly.
- Repetitions3 sets of 10
- How often3 times a week
Part 3: Stronger and steadier
Add these on the same three days when Part 2 feels easy. They make the hip stronger and steadier, for stairs, walking and balance.
Step-ups and step-downs
Strengthening the leg for stairs.
- Stand facing the bottom step of the stairs, holding the rail.
- Step up with your painful leg, keeping your hips level and the knee in line with your toes, and bring the other foot up beside it.
- Step back down with the other leg first, lowering yourself slowly on your painful leg.
- Repetitions3 sets of 10
- How often3 times a week
Lifting the leg, lying on your side
A harder exercise for the muscles at the side of the hip.
- Lie on your side with your painful hip on top, and your bottom knee bent to keep you steady.
- Keeping your top leg straight and in line with your body, lift it a short way towards the ceiling. Do not let it drift forwards.
- Hold, then lower it slowly. When this is easy, add an ankle weight.
- Hold5 seconds
- Repetitions3 sets of 10
- How often3 times a week
Standing on one leg
Improving your balance, and the control of the hip.
- Stand on your painful leg, keeping your hips level, and lightly hold on to a kitchen worktop.
- Keep your balance for up to 30 seconds.
- As it gets easier, hold on with just one finger, then let go, keeping the worktop within reach.
- HoldUp to 30 seconds
- Repetitions3 to 5
- How often3 times a week
Sport and work
Sport: walking, cycling and swimming are usually comfortable. Running, and sports with jumping, twisting or sudden stops, put more load on the hip. Your return to sport is built up gradually, and planned separately for each person, depending on the sport and what it demands of your hip.
Work: most people can keep working. Jobs with a lot of heavy lifting, climbing, squatting or standing load the hip the most, and may need adjusting for a while. Heavy work is built up gradually, and planned separately for each person, depending on your work and what it demands of your hip.
What to expect
Osteoarthritis pain often comes and goes. Exercise and the other measures described here will not cure it, and how much they help varies from person to person, but they help many people to stay active, and they are worth keeping up for good.
When an operation may help
A total hip replacement is a very successful operation for hip osteoarthritis. It replaces the worn ball and socket with new, smooth parts, and for most people it eases the pain and makes walking and everyday life much easier.
It may be considered when the pain and stiffness become severe, keep you awake at night, or stop you doing everyday things despite these measures: there is then no benefit in putting up with severe pain for a long time.
Trying exercises and the other measures first does not rule out a hip replacement later. Dr Malik will discuss whether an operation is right for you, and when.
Take it with you

Hip Osteoarthritis
