Plantar Fasciitis: Non-operative Management
For pain under the heel, where the plantar fascia joins the heel bone
Plantar fasciitis is the most common cause of pain under the heel. It usually gets better without an operation, with exercises, supportive footwear and time, although it often takes several months. This page explains what it is and what helps, and sets out the exercises step by step.
Understanding plantar fasciitis
The plantar fascia is a thick, strong band of tissue under the foot. It runs from the heel bone to the base of the toes, supports the arch, and works like a spring each time you take a step.
In plantar fasciitis, the plantar fascia becomes painful and thickened where it joins the heel bone. Despite its name, it is not mainly an inflammation: it is overload, when the tissue is asked to take more strain than it can cope with, and it slowly becomes worn and weakened.
The typical pain is under the heel, worst with the first steps in the morning or after sitting for a while. It often eases after a few minutes of walking, then returns after long spells of standing or walking. It is most common between the ages of 40 and 60, and more likely if you are overweight, stand for long hours on hard floors, walk barefoot or in flat, thin-soled footwear, have tight calf muscles, or have suddenly increased your walking or running.
It is usually diagnosed from your symptoms and an examination, and scans are not usually needed. A heel spur (a small bony point on the heel bone) is often seen on an X-ray, but many people without heel pain have one too. It usually does not need to be removed: removing it often does not stop the pain from coming back.
What helps most
- Stretching the sole of the foot and the calf, every day, and especially before your first steps in the morning.
- Strengthening the calf and foot muscles, which helps the heel take load again.
- Supportive, cushioned footwear, and a heel cushion or an insole that supports the arch.
- Easing the load on the heel for a while, and keeping to a healthy weight.
- Patience: the heel usually settles, but slowly, and these measures work best when you keep them up.
Footwear and insoles
- Wear shoes or sandals with a cushioned sole, a slightly raised heel and support under the arch, indoors as well as outdoors.
- While the heel is painful, avoid walking barefoot on hard floors, and flat, thin chappals or flip-flops.
- Keep a pair of cushioned slippers by the bed, and put them on before your first steps in the morning.
- A ready-made heel cushion or arch-supporting insole, from a pharmacy or sports shop, is enough for most people. Custom-made insoles are sometimes needed, for example if you have flat feet: Dr Malik will tell you if one may help.
- Replace shoes whose soles are worn thin or flattened.
Everyday tips
- If you are overweight, losing weight reduces the load on your heels, and helps them settle.
- If work or household chores keep you on your feet, for example long hours in the kitchen, stand on a cushioned mat where you can, wear supportive footwear, and take short sitting breaks.
- Stay active, but for a while swap long walks, running and jumping for cycling or swimming, and build walking back up gradually.
Easing the pain
- Rolling the sole of the foot over a cold bottle of water, or holding an ice pack wrapped in a thin towel on the heel for up to 15 minutes, can ease the pain.
- Painkilling or anti-inflammatory tablets or gels, taken as prescribed by Dr Malik for a short time, can ease the pain enough to keep you active.
If the pain does not settle, or you are worried about your heel, contact the clinic on +91 99197 03477.
Contact the clinic if
- The pain is no better after about 3 months of doing the exercises regularly.
- Both heels are painful, especially if your back is stiff in the mornings or other joints are swollen.
- You have burning, tingling or numbness in the heel or the sole of the foot.
- The pain comes at night or at rest, rather than mostly when you stand and walk.
Contact the clinic urgently if
- You felt a sudden snap or tearing pain under the foot, followed by bruising or swelling.
- The heel becomes hot, red and swollen, or you feel feverish or unwell.
- After a fall, or a big increase in walking or running, the heel hurts when it is squeezed from the sides, or you cannot put weight on it.
- You have diabetes and notice a wound, a blister, redness or swelling on the foot.
Clinic: +91 99197 03477.
How to do the programme
- Do Part 1 every day. Do the first stretches before you get out of bed in the morning, and again before you stand up after sitting for a long time.
- Do Part 2 every other day: the muscles get stronger on the rest days. When Part 2 feels easy, move on to Part 3 on the same days.
- Move slowly and with control. In the strength exercises, the last few repetitions should feel like hard work.
- If both heels are painful, do the exercises with both feet.
- Give it time: it usually takes several weeks of regular exercise before the heel feels better, and it keeps improving over the following months.
Before you begin
- If you have heart or lung problems, or problems with your balance, check with Dr Malik before you start.
- Some discomfort during the exercises is normal, and does not mean you are harming your heel. Do a little less next time if the pain is much worse than usual during an exercise, has not settled within about 2 hours, or the heel is more painful the next morning; then build up again gradually.
- When you exercise standing up, hold on to a kitchen worktop, a rail or a sturdy chair.
- Follow these exercises as they are written unless Dr Malik tells you otherwise.
- This information does not replace individual advice. If it differs from what you were told in clinic, follow the advice you were given there.
Part 1: Stretching
Every day. Stretch before your first steps in the morning, and whenever you get up after sitting for a while.
Stretching the sole of the foot
Stretching the plantar fascia itself: the most helpful stretch for morning heel pain.
- Sit and cross your painful foot over your other knee.
- With one hand, pull your toes back towards your shin until you feel a stretch in the arch of your foot.
- With the other hand, feel the band under the arch: it should feel firm, like a tight string. Hold, then relax.
- Hold10 seconds
- Repetitions10
- How often3 times a day
Stretching the calf with a towel
Stretching the calf before you stand: a good one to do in bed in the morning.
- Sit with your painful leg straight out in front of you.
- Loop a towel around the ball of your foot and hold the ends.
- Gently pull the towel towards you until you feel a stretch in the calf. Hold, then relax.
- Hold30 seconds
- Repetitions3
- How oftenMorning and evening
Stretching the calf against a wall
Stretching both muscles of the calf, which pull on the heel.
- Stand facing a wall with your hands on it, your painful leg behind and the other leg in front.
- Keeping the back heel on the floor and the knee straight, lean towards the wall until you feel a stretch in the calf. Hold.
- Then bend the back knee a little, keeping the heel down, to stretch lower in the calf. Hold.
- Hold30 seconds each
- Repetitions3
- How oftenTwice a day
Rolling the sole of the foot
Easing tightness and pain in the sole of the foot.
- Sit on a chair with a bottle of water, or a tennis ball, under the sole of your painful foot.
- Roll the foot slowly back and forth over it, from the heel to the base of the toes, pressing firmly but not painfully.
- A bottle of water cooled in the fridge also eases the pain.
- Time1 to 2 minutes
- How oftenTwice a day
Part 2: Strengthening
Every other day. Start with 1 set of each and build up to 3 sets. Move on to Part 3 when these feel easy.
Heel raises
Strengthening the calf, which shares the load with the plantar fascia.
- Stand holding on to a kitchen worktop, with your feet hip-width apart.
- Rise up slowly onto your toes, hold, then lower slowly.
- When this is easy, do it on your painful foot only.
- Hold2 seconds
- Repetitions3 sets of 15
- How oftenEvery other day
Scrunching a towel with your toes
Strengthening the small muscles under the foot, which support the arch.
- Sit on a chair with a small towel flat on the floor under your painful foot.
- Keeping your heel on the floor, curl your toes to scrunch the towel towards you.
- Spread it out again, and repeat.
- Repetitions3 sets of 10
- How oftenEvery other day
Standing on one leg
Improving the balance and control of the foot and ankle.
- Stand on your painful foot, knee slightly bent, lightly holding on to a kitchen worktop.
- Keep your balance for up to 30 seconds.
- As it gets easier, hold on with just one finger, then let go, keeping the worktop within reach.
- HoldUp to 30 seconds
- Repetitions3 to 5
- How oftenEvery other day
Part 3: Heavier strengthening
Every other day, in place of the heel raises in Part 2, once you can do 3 sets of 15 heel raises on your painful foot. Keep doing Part 1 every day.
Heavy heel raises on a step
Slow, heavy strengthening that loads the plantar fascia: it often eases the pain sooner.
- Stand on your painful foot on the bottom step of the stairs, holding the rail, with a rolled-up towel under your toes so that they bend upwards. Let the heel hang over the edge.
- Rise up onto your toes slowly, counting 3 seconds; hold for 2 seconds at the top; then lower slowly for 3 seconds, until the heel is a little below the step.
- The last few raises of each set should be hard. When 3 sets of 12 feel easy, wear a backpack with some books in it, and add weight gradually.
- Repetitions3 sets of 12
- Rest2 minutes between sets
- How oftenEvery other day
Sport and work
Sport: you can usually carry on with non-impact sports, such as swimming and cycling, and they are a good way to keep fit while the heel settles. Running, and sports with jumping or sudden changes of direction, load the heel the most: your return to them is built up gradually, and planned separately for each person, depending on the sport and what it demands of your foot.
Work: most people can keep working. Jobs with long hours of standing or walking, or carrying heavy loads, put the most strain on the heel, and may need adjusting for a while. Heavy work is built up gradually, and planned separately for each person, depending on your work and what it demands of your feet.
What to expect
Plantar fasciitis usually gets better, but slowly: most people improve over several months, though it can take a year or more, and it tends to take longer when both heels are painful. The exercises and other measures described here help it settle, and keep the pain down while it does.
If the pain does not settle
Most heel pain improves with the measures described here. If yours has not improved after about 3 months of doing them regularly, Dr Malik may suggest one of the treatments below, alongside the exercises.
A steroid injection into the heel, given in clinic, can ease the pain for some weeks, and make it easier to keep up the exercises. It does not cure the problem, and the pain can come back as the injection wears off. Usually one injection is enough; sometimes a second one is given after 6 months. Because there is a small risk that the plantar fascia can tear, or that the cushioning fat pad under the heel can thin, injections are not given more often than this. For a week or two after an injection, avoid running, jumping and long walks.
Shockwave therapy, which a physiotherapist can offer, sends pulses of energy into the heel from a machine held against the skin, usually in a few short sessions a week apart. It can help some people whose pain has lasted many months. It can be uncomfortable during the treatment, and the heel may be sore or red for a day or two.
PRP injections (prepared from a sample of your own blood) are not usually offered: some studies suggest they may help, but most are small and of mixed quality, and the evidence is not strong enough to recommend them routinely. If you would like to try one, Dr Malik can discuss it with you.
When an operation may help
An operation is rarely needed. If the pain remains severe after many months of these treatments, and your calf muscle is tight, Dr Malik may discuss an operation to release the tight calf muscle (a gastrocnemius release). It is kept for the few people who do not get better.
Trying exercises and the other measures first does not rule out an operation later. Dr Malik will discuss whether an operation is right for you.

Plantar Fasciitis: Non-operative Management
