Understanding tennis elbow

Tennis elbow is pain on the outer side of the elbow, where the muscles that lift your wrist and fingers are attached to the bone by a tendon. If this tendon is overloaded, for example by a lot of gripping, lifting or repeated wrist movements, it can become sore and less able to cope with load.

Despite its name, you do not need to play tennis to get it. It is most common between the ages of about 40 and 60, and in people whose work or hobbies involve forceful or repeated gripping, such as using tools, carrying heavy bags, cooking, typing or using a mouse.

It causes pain on the outer side of the elbow, which can spread down the forearm. It is usually worse when you grip, lift with the palm facing down, turn a door handle or a key, or shake hands, and your grip may feel weak.

It usually gets better, but slowly: most people are better within a year. Exercises that gradually strengthen the tendon are the treatment with the best evidence. The exercises are set out below.

Everyday tips

  • Keep using your arm, but for now ease off the activities that bring on the pain. Resting the arm completely does not help the tendon recover.
  • Lift with your palm facing up, and keep your elbow bent and close to your body. Lifting with the palm facing down strains the sore tendon most.
  • Use both hands to lift and carry where you can, and carry bags on your shoulder or close to your body.
  • Avoid gripping tightly for long periods. Thicker handles on tools, pens and kitchen utensils need a lighter grip.
  • Take short breaks from repeated tasks such as typing, using a mouse or tools, or chopping.

Easing the pain

  • A cold compress, such as an ice pack wrapped in a thin towel, can ease the pain after activity.
  • Painkillers, taken as advised by your doctor, can make it easier to keep up the exercises. An anti-inflammatory gel rubbed onto the elbow may also help for a few weeks.
  • A tennis elbow strap, a band worn just below the elbow, can ease the pain during activities for some people, but it does not cure the problem.
  • Steroid injections can give good relief from the pain in the short term, but it is not certain that they help in the long term.
  • Platelet-rich plasma (PRP) injections, prepared from a small sample of your own blood, can give excellent relief from the pain in the short term; whether they help in the long term is not yet known. Dr Malik will discuss whether an injection is right for you.

If the pain is stopping you from exercising, or you are worried about your elbow, contact the clinic on +91 99197 03477.

Contact the clinic if

  • You have numbness or tingling in your hand or fingers.
  • Your hand or grip becomes weak.
  • Your elbow swells, or locks or catches when you move it.
  • The pain began after a fall or an injury.
  • The pain is severe at night, or does not ease with rest.

Before you begin

  • Start only when Dr Malik or your physiotherapist has told you to.
  • Some discomfort during the lowering exercises is expected and fine, as long as it settles within 30 minutes and is no worse the next day. If it is worse, use a lighter weight or do fewer repetitions next time, then build up again gradually.
  • Stop, and contact the clinic, if the pain becomes sharp or severe, or you notice numbness or tingling in your hand.
  • Follow these exercises as they are written unless Dr Malik tells you otherwise.
  • This information does not replace individual advice. If it differs from what you were told in clinic, follow the advice you were given there.

How to do the programme

  • Do the stretches and the lowering exercises every day, at a time when you are not rushed.
  • Keep going for at least 12 weeks, even when the pain eases: tendons take months to strengthen.
  • Increase the weight by about 0.5 kg when you can do all 3 sets with only mild discomfort that settles within 30 minutes.
  • Keep a diary of the weight and the repetitions you do, so that you can see your progress.

Part 1: Stretches

Do the stretches before and after the lowering exercises, and whenever your forearm feels tight during the day. A stretch should feel like a gentle but definite pull, never sharp pain.

01

Stretching the back of the forearm

Stretching the muscles and the tendon at the outer side of the elbow.

  1. Stand or sit tall, and hold your sore arm out in front of you at shoulder height, with the elbow straight and the palm facing down.
  2. With your other hand, gently bend your wrist down, so that your fingers point towards the floor, until you feel a stretch along the back of your forearm. Turning the hand slightly towards the little-finger side adds to the stretch.
  3. Hold, then relax.
  4. If this hurts at first, do it with your elbow bent and your arm by your side, and straighten the elbow as the stretch becomes easier.
  • Hold30 seconds
  • Repetitions3
  • WhenBefore and after the lowering exercises
02

Stretching the front of the forearm

Stretching the muscles on the other side of the forearm, which do the gripping.

  1. Hold your sore arm out in front of you at shoulder height, with the elbow straight and the palm facing up.
  2. With your other hand, gently bend your wrist back, so that your fingers point towards the floor, until you feel a stretch along the front of your forearm.
  3. Hold, then relax.
  • Hold30 seconds
  • Repetitions3
  • WhenBefore and after the lowering exercises

Part 2: Lowering exercises

Here the sore tendon works while the muscle lengthens: you lower slowly, and your other hand does the lifting. This is called eccentric exercise; research shows that it eases pain and builds strength. Do them once a day, after the stretches.

03

Lowering the wrist with a weight

The main exercise: strengthening the tendon at the outer side of the elbow.

  1. Sit beside a table with your sore forearm resting on it, palm facing down, and your hand over the edge.
  2. Hold a light weight, such as a full 500 ml water bottle. Use your other hand to lift your wrist up as far as it comfortably goes.
  3. Take your other hand away, and lower the weight slowly, counting to 4, until your wrist is bent down.
  4. Use your other hand again to lift the wrist back up, and repeat. The sore arm only lowers; it never lifts.
  • Repetitions3 sets of 15
  • Rest1 minute between sets
  • Weight0.5 kg, building up to about 2 kg
04

Turning the forearm slowly

Strengthening the muscle that turns the palm up, which also starts at the outer side of the elbow.

  1. Sit with your sore forearm resting on a table, or on your thigh, with your hand over the edge. Hold a hammer by its handle, with your thumb pointing up.
  2. Slowly turn your hand so that the palm faces down, letting the hammer lean over in a controlled way, counting to 4.
  3. Use your other hand to bring the hammer back to the start, and repeat. Holding the handle close to the metal head makes this easier.
  • Repetitions3 sets of 15
  • Rest1 minute between sets
  • Next stepHold the handle nearer its end
05

Twisting a flexible bar (if you have one)

Another way of lowering under control, using a flexible rubber exercise bar.

  1. Hold the bar upright in front of you in your sore hand, with that wrist bent fully back.
  2. Grip the top of the bar with your other hand, palm facing away from you, and twist the bar by bending that wrist forward, while your sore wrist stays bent back.
  3. Keeping the twist, straighten both elbows, so that the bar is held out in front of you, level with the floor.
  4. Slowly let the bar untwist by letting your sore wrist bend forward, counting to 4.
  • Repetitions3 sets of 15
  • Rest1 minute between sets
  • Next stepA stiffer bar

Part 3: Getting stronger

Start these when the lowering exercises have become easy, usually after about 6 weeks. Keep doing the stretches.

06

Lifting and lowering the wrist

Building strength once the lowering exercise has become easy.

  1. Set up as in exercise 03, with your forearm on the table and a weight in your hand, palm facing down.
  2. Lift the weight by bending your wrist up, counting to 3, then lower it slowly, counting to 3. This time the sore arm does both.
  3. Choose a weight that makes the last few repetitions hard work, with no more than mild discomfort.
  • Repetitions3 sets of 15
  • Rest1 minute between sets
  • Instead ofExercise 03
07

Squeezing a soft ball

Rebuilding grip strength for everyday tasks.

  1. Hold a soft ball, or a rolled-up towel, in your sore hand, with your forearm on a table.
  2. Squeeze it as firmly as you comfortably can, and hold.
  3. Relax slowly, and repeat.
  • Hold5 seconds
  • Repetitions10
  • How oftenOnce a day

Getting back to sport

Your return to sport is built up gradually, and planned separately for each person, depending on the sport and what it demands of your elbow.

What to expect

Tendons recover slowly. It can take 6 weeks or more before you notice a change, so keep going. Most people with tennis elbow are better within a year, and the exercises help you get back to your normal activities with a stronger arm.

If your elbow is not improving, or is getting worse despite the exercises, contact the clinic.

If the pain does not settle: surgery

For a small number of people, the pain carries on for many months despite doing the exercises properly, and other treatments have not helped either. For them, an operation can be considered: when everything else has failed, surgery has a good success rate.

The operation is usually done as a day case, so you go home the same day. Through a small cut over the outer side of the elbow, or by keyhole surgery, the surgeon removes the damaged part of the tendon and may release it from the bone, so that the area can heal.

Afterwards, the elbow is rested for a short time, and then gentle movement, stretches and, later, strengthening exercises are built up step by step. Most people are back to everyday activities within a few weeks, and to heavier work and sport after a few months. Dr Malik will discuss whether surgery is right for you, and what it involves.

Tennis Elbow
