Neck Pain and Cervical Spondylosis
Including arm pain from the neck (cervical radiculopathy)
Neck pain is very common, and most flare-ups settle with simple treatment. This page explains cervical spondylosis and arm pain from the neck in plain words, and sets out the main treatment: exercises that strengthen the neck, mainly by pressing against your own hand without moving (isometric exercises), with gentle moving exercises, stretches and small changes to your posture. It also covers other treatments, and when to get help urgently.
Understanding cervical spondylosis and neck pain
Your neck is made up of seven small bones, stacked on top of each other, with discs between them that act as cushions, and small joints at the back. Muscles and ligaments hold the neck steady and move it. The spinal cord runs down inside the bones, and nerves branch off it, leaving through small openings between the bones to reach your shoulders, arms and hands.
As we get older, the discs and joints of the neck change, much as hair turns grey: the discs lose some height, and small bony spurs form at the edges of the bones. This wear and tear is called cervical spondylosis. It is very common, and shows up on the X-rays and scans of many people with no neck pain at all. So it does not always explain the pain, and it does not mean that your neck is weak or that moving it will harm it.
Pain from the neck itself is usually an ache in the neck, which may spread to the back of the head, the shoulders or between the shoulder blades. The neck often feels stiff, especially in the morning or after sitting in one position for a long time. Some people also get headaches that start at the back of the head.
Sometimes a nerve in the neck becomes irritated or pinched where it leaves the spine, for example by a bulging disc or a bony spur. This causes pain that spreads from the neck down the arm, often with tingling, numbness or weakness in part of the arm or hand. This is called cervical radiculopathy.
Exercises that strengthen the neck, mainly isometric exercises, in which the muscles work while the head stays still, together with gentle movements, stretches and small changes to your posture, are the main treatment. They are set out below.
Posture and everyday life: small changes that help
- There is no single perfect posture, and posture is not usually the whole cause of neck pain. But long spells in one position, especially with the head bent forward, often make a sore neck ache more. Change position often, and take a short break every 20 to 30 minutes to move your neck and shoulders.
- At a desk, sit back in the chair with your lower back supported, your feet flat on the floor, your shoulders relaxed, and your forearms resting on the desk or the armrests.
- Raise the screen so that its top is about level with your eyes, about an arm’s length away. With a laptop, use a separate keyboard and mouse, and raise the laptop on a stand or a pile of books.
- Hold your phone or tablet up nearer eye level, rather than bending your head down to it for long spells.
- Prop up what you read, on a book stand or a cushion. Avoid long spells of reading or watching a screen in bed with your head pushed forward on pillows.
- Sleep on your back or your side, on one pillow that keeps your head level with your spine: not so high that it tips forward, nor so low that it drops. Sleeping on your front keeps the neck twisted for hours, so it is best avoided.
- Carry bags close to your body, with the load shared between both hands, or use a backpack on both shoulders. Avoid carrying heavy loads on your head.
- At the hairdresser or barber, do not let anyone tip your head far back over a basin, or twist or ‘crack’ your neck.
- Keep active: walking on most days, and a good night’s sleep, are good for your neck and your general health.
Exercises 08, 11 and 12 also help you to sit and stand tall more easily.
Easing the pain
- Heat, such as a heat pack or a hot-water bottle wrapped in a towel, can ease the pain and stiffness. Use it for up to 20 minutes at a time.
- Painkillers, and medicines for nerve pain if you have arm pain, taken as prescribed by Dr Malik, can make it easier to keep moving and to do the exercises.
- A neck collar is not usually needed. Dr Malik advises one only for very severe pain in the early, acute stage, and then only for a short time.
- A physiotherapist can help with short-term pain relief. Options include TENS (a small machine that sends gentle electrical pulses through pads on the skin), infrared heat, ultrasound therapy, hands-on treatment such as gentle mobilisation, and, for arm pain from a pinched nerve, traction (a gentle, steady pull on the neck). Discuss with your physiotherapist which of these suit you. They ease the pain for a while; the exercises are what help in the long run.
- For severe arm pain from a pinched nerve that is not settling, an injection around the nerve in the neck can ease the pain for a while. Dr Malik does not give these injections himself, but if an injection might help you, he can refer you to a specialist who does.
If the pain is stopping you from moving or exercising, or you are worried about your neck, contact the clinic on +91 99197 03477.
Contact the clinic if
- You have a fever, feel unwell, or are losing weight without trying.
- You have had cancer in the past, and the neck pain is new.
- The pain began after a fall or an injury, or you have thin bones (osteoporosis) or rheumatoid arthritis, or take steroid tablets.
- The pain is severe and constant, including at night, and does not ease when you lie still or change position.
- You have numbness or weakness in your arm or hand, or the arm pain is severe and is not settling.
- Your pain is not improving after 6 weeks, or is getting worse despite the exercises.
Contact the clinic urgently if
- Your hands become clumsy, for example you drop things, or find buttons or writing difficult, or you have numbness or tingling in both hands.
- Your walking becomes unsteady, or your legs feel weak or stiff.
- Weakness in your arm or hand comes on suddenly, or is getting worse.
- You lose control of your bladder or bowels.
These can be signs of pressure on the spinal cord in the neck, which needs prompt treatment. Clinic: +91 99197 03477. Go to the nearest emergency department straight away if neck pain or a severe headache comes on suddenly with dizziness, double vision, slurred speech, difficulty swallowing, or weakness or numbness of the face, arm or leg.
How to do the programme
- Each card says how often to do it: most exercises 3 times a day, the stretches twice a day, and the exercises done lying down or with a band once a day. Do them at a time when you are not rushed.
- Sit on a firm chair for most exercises. For the chin nod (06), lie on a mat or a folded blanket on the floor, or on a firm bed if getting down to the floor is difficult.
- Start gently, and build up as the exercises become comfortable: press a little harder, move a little further, or do a few more repetitions.
- Keep going for at least 12 weeks, even when the pain eases. Afterwards, keep up the isometric exercises and the rowing a few times a week: the benefit fades if you stop.
- For nerve pain that spreads down your arm, Dr Malik may also advise nerve glides: do them as set out on his Nerve Glides page.
Before you begin
- Start only when Dr Malik or your physiotherapist has told you to.
- Move slowly and smoothly, and never force or jerk your neck. Some discomfort during the exercises is fine, as long as it settles within 30 minutes and is no worse the next day; if it is worse, do fewer repetitions or smaller movements next time.
- Watch your arm symptoms. If an exercise makes pain, tingling or numbness spread further down your arm, stop it and leave it out. If the arm pain eases, or moves back up towards your neck, that is a good sign.
- If you feel dizzy or sick, or your vision blurs, stop and sit quietly; if it does not pass within a few minutes, contact the clinic.
- If you have been told that the spinal cord in your neck is squeezed (myelopathy), do these exercises only if Dr Malik advises.
- Follow these exercises as they are written unless Dr Malik tells you otherwise.
- This information does not replace individual advice. If it differs from what you were told in clinic, follow the advice you were given there.
Part 1: Isometric exercises (the main exercises)
Here your head stays still while the neck muscles work: you press your head against your own hand, which stops it from moving. This builds strength without moving the sore neck, and often eases the pain. Start with a gentle press and build up over the weeks, as long as it does not hurt. Keep breathing normally; do not hold your breath.
Chin tuck
Drawing the head straight back. This is the starting position for the pressing exercises.
- Sit upright on a firm chair, looking straight ahead, with your shoulders relaxed.
- Gently draw your chin and head straight back, as if making a double chin, keeping your eyes level. Do not tip your head up or down.
- Hold, then relax.
- Hold5 seconds
- Repetitions10
- How often3 times a day
Pressing forward
Strengthening the muscles at the front of the neck.
- Sit upright with your chin slightly tucked in, looking straight ahead.
- Place the palm of one hand, or both hands, on your forehead.
- Press your forehead gently into your hand, while your hand stops your head from moving.
- Hold, then relax slowly.
- Hold10 seconds
- Repetitions5
- Rest5 seconds between
- How often3 times a day
Pressing back
Strengthening the muscles at the back of the neck.
- Sit upright with your chin slightly tucked in.
- Clasp your hands behind your head.
- Press your head back into your hands, which stop it moving; look straight ahead.
- Hold, then relax slowly.
- Hold10 seconds
- Repetitions5
- Rest5 seconds between
- How often3 times a day
Pressing to the side
Strengthening the muscles at the sides of the neck.
- Sit upright with your chin slightly tucked in.
- Place your right palm on the right side of your head, just above the ear.
- Press your head into your hand, as if to tilt it sideways, while the hand stops it.
- Hold, then relax slowly. Repeat on the left side.
- Hold10 seconds
- Repetitions5 to each side
- Rest5 seconds between
- How often3 times a day
Pressing into a turn
Strengthening the muscles that turn the head.
- Sit upright with your chin slightly tucked in.
- Place your right palm on the right side of your face, at the temple.
- Try gently to turn your head to the right, while your hand stops it from turning.
- Hold, then relax slowly. Repeat on the left side.
- Hold10 seconds
- Repetitions5 to each side
- Rest5 seconds between
- How often3 times a day
Chin nod, lying down
Training the deep muscles at the front of the neck, which hold it steady.
- Lie on your back with your knees bent, and your head on a thin pillow or a folded towel, so that your face is level.
- Gently nod your head, as if saying ‘yes’, tucking your chin in slightly, so that the back of your head slides up the pillow. Do not lift your head or press it down.
- Hold, breathing normally, then relax.
- You should feel the deep muscles at the front of your neck working gently, not the strong muscles at the sides.
- Hold10 seconds
- Repetitions10
- How oftenOnce a day
Part 2: Moving (isotonic) exercises
These ease stiffness and build strength through movement. Move slowly and comfortably.
Neck movements
Moving the neck gently in each direction, to ease stiffness.
- Sit upright on a firm chair, with your chin slightly tucked in.
- Turning: slowly turn your head to look over one shoulder, then the other.
- Tilting: slowly tilt your head to bring one ear towards your shoulder, then the other, without lifting your shoulder.
- Looking down and up: slowly bring your chin to your chest, then look up as far as is comfortable; if this makes you dizzy or brings on arm pain, look up only a little.
- Repetitions10 in each direction
- How often3 times a day
Rowing with an exercise band
Strengthening the muscles between the shoulder blades, which help you sit and stand tall.
- Loop an exercise band (a long elastic band) around a firm post at about chest height, and hold one end in each hand, with your arms in front of you.
- Sit or stand tall, with your chin slightly tucked in and your shoulders down.
- Pull the band towards you, squeezing your shoulder blades together and down.
- Hold for 2 seconds, then let your arms go forward slowly. No band? Do it without one.
- Repetitions2 sets of 10
- Rest1 minute between sets
- How oftenOnce a day
- Next stepA stronger band
Part 3: Neck and shoulder stretches
Stretches ease tight muscles in the neck, shoulders and upper back, and the last two help you to sit and stand tall. Stretch gently until you feel a pull, never pain, and do not bounce.
Side neck stretch
Stretching the muscles at the side of the neck and the top of the shoulder.
- Sit upright and hold the edge of the chair seat with your right hand, to keep that shoulder down.
- Tilt your head to the left, bringing your left ear towards your left shoulder, until you feel a gentle stretch on the right side of your neck.
- For a little more stretch, rest your left hand on your head and let its weight help. Do not pull.
- Hold, then come back slowly, and repeat on the other side.
- Hold20 to 30 seconds
- Repetitions3 to each side
- How oftenTwice a day
Neck to armpit stretch
Stretching the muscle that runs from the side of the neck to the top of the shoulder blade.
- Sit upright and hold the edge of the chair seat with your right hand.
- Turn your head halfway to the left, then bend it forward, as if looking down towards your left armpit, until you feel a gentle stretch at the back of your neck on the right.
- For a little more stretch, rest your left hand on the back of your head and let its weight help. Do not pull.
- Hold, then come back slowly, and repeat on the other side.
- Hold20 to 30 seconds
- Repetitions3 to each side
- How oftenTwice a day
Chest stretch in a doorway
Opening up the front of the chest, so that the shoulders and head can sit back more easily.
- Stand in a doorway and rest your forearms on either side of the door frame, with your elbows a little below shoulder height.
- Step forward with one foot, until you feel a gentle stretch across the front of your chest and shoulders. Keep your head tall and your back straight.
- Hold, then step back.
- If your arms tingle, lower your elbows; if the tingling carries on, leave this stretch out.
- Hold20 to 30 seconds
- Repetitions3
- How oftenTwice a day
Upper back stretch over a chair
Easing stiffness in the upper back, which takes strain off the neck.
- Sit on a firm chair with a back that reaches about the level of your shoulder blades.
- Clasp your hands behind your head to support it, with your elbows pointing forward.
- Gently lean back over the top of the chair back, so that your upper back arches. Keep your head supported in your hands, and do not let it drop back.
- Hold, then sit up again.
- Hold5 seconds
- Repetitions10
- How oftenTwice a day
Work, sport and lifting
Stay at work, or go back as soon as you can, on light duties at first if needed: you do not need to wait until the pain has gone. Avoid heavy lifting at first, and loads carried overhead or on your head. Heavy lifting, sport and the gym are built up gradually, and planned separately for each person, depending on your work or sport and what it demands of your neck. Walking, swimming and an exercise bike can usually be started early. Yoga can help, but avoid poses that put weight on your head or neck, such as the headstand, the shoulder stand and the plough. Drive only when you can turn your head comfortably to check over your shoulder.
What to expect
Neck pain often eases a lot within a few weeks; arm pain from a pinched nerve can take longer, often a few months. Most people get better without an operation.
Neck pain often comes and goes. A flare-up does not mean that your neck is damaged: keep moving, ease off the harder exercises for a few days, then build up again.
If the pain does not settle: surgery
Most people with neck or arm pain do not need an operation; for neck pain from wear and tear alone, one is rarely needed.
An operation may be needed when your symptoms, examination and scans together show pressure on the spinal cord, a large disc bulge, severe pressure on a nerve, or signs that the spinal cord is being affected (myelopathy). It can also help when a pinched nerve causes severe arm pain that does not settle after several weeks of treatment, or when the arm or hand is becoming weak.
Dr Malik does not operate on the spine himself. If an operation might help, he will refer you to a spine surgeon or a neurosurgeon, who will discuss whether it is right for you and what it involves.

Neck Pain and Cervical Spondylosis
